If you prefer to call our office and we miss your call, please leave a detailed message including your child's name, date of birth and insurance carrier if you are inquiring to make an appointment, and we will respond to your inquiry as soon as possible.  This information will help us look up your child's authorization or prescription if it was sent to us by your child's insurance or doctor's office.  Due to the volume of phone calls we receive at a time and because we value your time, we want to make sure we are prepared for you when we return your call. 

When you schedule an initial evaluation with us, you will be emailed link(s) to fill out intake e-forms which are important and necessary to complete for us in preparing your child's file and preparing the evaluating therapist for your child's first visit.   Completing the e-forms within the given timeline will guarantee and confirm your child's appointment.  Failure to complete the forms by the deadline will result in not confirming your appointment date and time, and will be given to the next family. 

Please note, regardless of your insurance a referral or prescription (for PPO) or an authorization (for CalOptima) is needed.
